By submitting this form, I agree to participate in the production of media (print, audio, video or other mediums) produced by Little Acts of Kindness, and hereby grant to their subsidiaries, affiliates, agents, successors and assigns the right and permission to record, use, publish, stream live, offer for sale to benefit the programmes of Little Acts of Kindness, or otherwise distribute this production.

I warrant and agree that I have the legal and mental capacity to contract and to enter into this agreement and that, in doing so, I will not violate the rights of any third parties. I agree that Little Acts of Kindness may use my name, likeness, image, voice, writing, music or other applicable medium for the commercial and non-commercial purposes of reproducing, preparing derivative works of, distributing, and presenting this project in any and all forms of media now in existence or to be created in the future.

I understand that I will receive no payment for my participation or for the use of my name, likeness, image, voice, music, writing or other medium of expression that may be included in the production. I agree to hold harmless and indemnify Little Acts of Kindness and its subsidiaries, affiliates, agents, successors and assigns against any and all claims of liability or infringement that arise out of or in relation to my participation in this production.

In consideration for my participation, Little Acts of Kindness grants me a limited, non-exclusive and royalty-free license to market, promote, and distribute the media or the media transcript in whole or in part for my own purposes.

This release shall be binding upon me and my heirs and legal representatives.
